| Massachusetts law requires that we request the name and address of each donor. |
| We also request that you include your occupation and employer information to assist with our record keeping. |
| Please note that the maximum contribution for an individual is $500 per person, per year. |
| Contributions from corporations are prohibited. |
| Contributions are not tax deductible. |
